A compact fortress sited on a raised oval mound, dominated by a large cylindrical keep with a high plastered drum and an upper wooden or glazed gallery; the keep joins rectangular stone ranges and buttressed masonry towers of local sandstone. Exterior walls show patched plaster over rough masonry, narrow arched windows and small embrasures. The southern and eastern faces step down a sheer 20-metre cliff, while moats ring the lower northern and western approaches. At prime the masonry and towers are whole, roofs intact.

The town of Ostroh belonged to the Ostrogski family from 1386, and it was the family's founder, Danylo Ostrozki, who reconstructed the earlier ruins into a castle on the city's Sudovyi Hill.
