'The Roman' — for a man from Rome, of Roman descent, or who had made the great pilgrimage to the city (a romeo, a Rome-pilgrim).
The history of Romano
Romano named the man marked by Rome: born there, descended from there, or returned from pilgrimage to the tombs of the apostles. The related Romeo carried the pilgrim sense specifically — a romeo was one who had walked to Rome, which is exactly how Shakespeare's hero came by his name.
